For each sports season, Wahkiakum High School athletes must be sure they have submitted all the paperwork listed below and met all the eligibility requirements before the first practice in order to participate in a sport during that season.

Checklist of Items Required for All Sports
- Scholastic Eligibility
- ASB Card
- $60 pay-to-participate fee for high school athletes, due no later than one one day prior to the first game/event (not the day of) in which the athlete is eligible and does not guarantee playing time.
- Read the Wahkiakum High School Activity and Athletic Code Handbook
- The WHS Athletic and Activities Handbook also includes the Wahkiakum Concussion Fact Sheet, the Sudden Cardiac Arrest Fact Sheet, and the Safety Warnings for the specific sports your athlete will be participating in.
- After reviewing all these materials in the Handbook, print out the All Sports Signature Form (athlete's and parent's signature required) which is found on page 1 of the Wahkiakum High School Activity and Athletic Code Handbook. By signing this form, your athlete is cleared to participate in every sport for the entire school year.
- If your athlete plans to participate in two sports, read and sign the Dual Sports Participation Policy and Signature Form (Athlete's and parent's signature required.)
- Physical Exam Form, Medical Insurance, and Emergency Contact Info (Physician's, athlete's, and parent's signature required)
Student Accident and Health Insurance
All student athletes are required to have health insurance coverage.
For those who do not have medical insurance, Wahkiakum School District makes available to parents a relatively low-cost Student Accident and Sickness Coverage for their children. Families without student athletes are also allowed to purchase this coverage, if they are so inclined. There is however, no requirement on the District's part for student non-athletes to have this coverage. The coverage is linked below.
